ROOT CAUSE FAILURE ANALYSIS

Course Overview

Root Cause Failure Analysis (RCFA) is an analysis process used to determine why a failure occurred and what needs to be done to prevent the failure from occurring again. Every organization has purchased assets to fulfill a specific purpose. When they no longer perform that purpose, they have failed and are either no longer useful to the organization or less useful than desired. In addition, some failures can have very catastrophic consequences. At this point they must be either repaired or replaced and the organization may need to rebuild their reputation. Both options are expensive, so it is to the organization’s advantage to determine why the failure occurred to avoid the consequences of the failure in the future. This interactive workshop is designed to introduce the attendees to the principles of RCFA so they can more effectively address asset failures after they occur. The attendees will understand the basic concepts and have the tools they need to work with their organization to perform RCFA analyses.
